FollowMyHealth is The Orthopaedic & Fracture Clinic’s patient portal, which provides our patients with access to their health information anywhere there is an internet connection. FollowMyHealth lets you access and manage your health information in a secure, online environment – 24 hours a day/ 7 days a week!

With FollowMyHealth you can:

  • Email a question to your physician, physical therapist, or care team through a secure messaging system
  • Send appointment requests
  • Receive reminders for upcoming appointments
  • Receive lab results
  • Receive clinical documentation.
  • Access online bill pay
  • Access your health information using your Android smartphone or Apple iPhone®, iPod touch® or iPad®

Enroll Your Children

To protect the privacy and confidentiality of health information according to state and federal laws, parents and guardians have full access to their child’s health information in FollowMyHealth from birth through age 12. From age 13 up to age 18, the child needs to give authorization for a parent to have access to their health information. Once the child turns 18, the connection to The Orthopaedic & Fracture Clinic and their FMH account is automatically severed. The proxy representative will retain access to all historical information contained in the account, but no new updates will occur and messaging will be disabled. To enroll children please email FMHsupport@ofc-clinic.com or call 507-386-6600.

Who can sign-up for FollowMyHealth?

If you are a patient over the age of 18, you can sign up for a FollowMyHealth account. For a minor child between the ages of birth to 18, please contact the clinic to receive information about our proxy account policy and sign-up.

Who do I call for help?

FollowMyHealth support is available to assist you 8am-5pm, Monday-Friday at 507-386-6600 or email support at FMHsupport@ofc-clinic.com.

Is FollowMyHealth secure?

Yes! FollowMyHealth is a secure Internet site which means your information is encrypted and therefore, always safe and protected. FollowMyHealth access is permitted only to authorized users that have been verified through an activation process. Members have a unique user ID and password that is known only to them.

Am I able to share my information or gain access to another person’s information?

To share your information or receive access to another person’s information, please complete the FollowMyHealth registration/proxy form and submit it to The Orthopaedic & Fracture Clinic by mail at the address listed on the form, fax it to 507-625-5971 attention FMH Support, or drop off at OFC. To obtain a form please email FMHsupport@ofc-clinic.com. The authorized representative may be required to submit copies of legal documents supporting his/her authority to act on the patient’s behalf.

Can I view all of my child’s health information?

To protect the privacy and confidentiality of health information according to state and federal laws, parents and guardians have full access to their child’s health information in FollowMyHealth from birth through age 12. For children age 13 up to 18, the minor child will have to sign the registration form stating they understand the proxy representative will have access to their medical information and they understand they can cancel their account by notifying The Orthopaedic & Fracture Clinic. The proxy representative will also sign the form giving The Orthopeadic & Fracture Clinic authorization to enroll the child in a FollowMyHealth account. On the child’s 18th birthday, the connection to The Orthopaedic & Fracture Clinic and their FollowMyHealth account is automatically severed. The proxy holder will retain access to all historical information contained in the FMH account, but no new updates will occur and messaging will be disabled.

To receive access to your child’s information, you must complete the FollowMyHealth registration/proxy form and submit to The Orthopaedic & Fracture Clinic by mail at the address listed on the form, fax it to 507-625-5971 – attention FMH Support, or drop off at OFC. To obtain a form please email FMHsupport@ofc-clinic.com.

What should I do if I don’t understand my health information or feel it is incorrect?

If you do not understand your health information in FollowMyHealth, call or message OFC for clarification. You may also discuss your questions with your physician, physical therapist, or care team during your next clinic visit.
